Liverpool Advice on Prescription Programme (APP) an innovative social prescribing service for health professional launched in 2014.
These "talking boxes" are partners or supporters of the new and exciting APP service being delivered by Citizens Advice services in Liverpool.
They show what the service is, how to access the service and the benefits of partnership working.
In the first six months of going live, this service has worked with health professional and vulnerable patients across Liverpool via 93 GP practices.  The APP team have dealt with 3057 enquiries , helped patients managed £1.8 million of debt (£1,815,118) and maximised patient income to the value of nearly £3.5 million (£3,473,226).

This programme is enjoying notable successes in terms of service delivery and partnership working.

United utilities funds a project worker to provide urgent support to people who are assessed as having priority debts that requires urgent attention, for example problems with illegal lenders, bailiff action, eviction notices.
